Travel Profile Form

This form is for the purpose of creating a new travel profile or updating a current travel profile.

This if the final step in the process of registering for a trip.

Travel Profile Instructions

If you're joining a trip for the first time coordinated by Urban Chic Travelistas:

Please submit a new travel profile so we can have your information on file to complete your registration and for future trips.

If you've previously taken a trip coordinated by Urban Chic Travelistas:

Type your first & last name and email address into the travel profile form. The travel profile form will pull up your name and ask you to authenticate your information by email. (see example below).

Once you verify your account, please send yourself the verification email so your information will autofill in the form. Once you verify/update your information, submit the form.

Please be sure to complete the ENTIRE travel profile. The following sections are the only parts you can leave blank:

Passport - Skip this section if you do not have a valid passport during the time of registration or if the passport will not be valid through the designated trip dates. You may submit your passport information at a later time by clicking on the Passport Information Form link in the menu above.

Trusted Traveler Numbers/Known Traveler Number - Skip this section if you do not have a TSA PreCheck or Global Entry number.

Please do not email, text, or DM us your information directly and instead utilize the secured form links provided so that your personal information is saved in an encrypted system.
